Why rest makes it worse
Cartilage doesn't have a blood supply. It's fed by joint fluid that moves when the knee moves. Resting a painful OA knee starves the remaining cartilage and lets quadriceps strength drop — quadriceps strength is the strongest single predictor of how fast an OA knee declines. Every week off your feet loses roughly 3–5% of quad strength in over-60s. The guideline that works: move daily, even when sore; stop only if pain is above 5/10 during an activity or above 3/10 the next morning. That rule keeps most OA knees active for another decade.
The four exercises that carry the load
1. Sit-to-stand from a firm chair — 3 sets of 10, twice a day. This is the single most important exercise. Use arms only if you must. 2. Wall squat — start at 30° of knee bend, hold 20 seconds, 5 reps. Progress depth only if pain-free the next morning. 3. Step-ups on the first stair — 10 each leg. Controlled down is more important than fast up. 4. Straight-leg raise — lying on the back, knee straight, lift leg to the height of the opposite knee, 15 reps each side. Five days a week. Add a light ankle weight once you can do 3 × 15 of straight-leg raises without pain. Tai chi at a Seremban Chinatown community centre counts as a bonus, not a substitute.
Weight off the knee is worth more than any supplement
Every kilogram of body weight loaded on a knee during stair descent is about four kilograms of joint force. Losing 5 kg is often worth more than the strongest OA supplement on the market. - X-ray severity and pain correlate surprisingly poorly. Plenty of 'severe' X-ray knees manage fine for another decade with rehab; plenty of 'mild' X-ray knees are miserable. - Are injections (steroid, PRP, hyaluronic acid) worth it?
- Steroid injections help short-term — useful to calm a flare so you can start rehab. PRP and hyaluronic acid have mixed evidence; we'd rather spend 8 weeks of rehab budget first. - Can I still cycle and swim?
- Yes, and you should. Cycling is ideal for OA knees — load is low, the joint moves through range, and the quads work. Swimming and water walking are also excellent. Running on tarmac is harder on an OA knee; we usually steer people toward walking briskly around Lake Gardens Seremban instead.
- How long until stairs stop hurting?
